Short answer: vitamin B12 comes almost only from animal foods, so a vegetarian Indian diet often supplies too little. Low B12 shows up as tiredness, pins and needles, mouth ulcers, pale skin and poor memory. A blood test confirms it. Healthy adults need 2.5 µg a day; most vegetarians do better with a regular supplement, and a real deficiency needs a doctor's dose.
Why B12 deficiency is common in vegetarians in India
Plants do not make B12. Dairy and eggs carry some, but often not enough for a full day, and boiling milk repeatedly lowers it further. Long-term metformin, antacids and acid reducers (PPIs), age over 50 and gut conditions all reduce absorption. Indian studies have found low B12 in a large share of adults, vegetarians most of all.
Vitamin B12 deficiency symptoms
- Tiredness and breathlessness from anaemia
- Pins and needles or numbness in hands and feet
- Mouth ulcers, a sore red tongue
- Pale or slightly yellow skin
- Poor memory, low mood, irritability
- Darkening of skin over knuckles in some people
Nerve symptoms can become permanent if a deficiency runs for long, so do not wait on them.
Vitamin B12 normal range
Serum B12 below about 200 pg/mL is usually read as deficient and 200 to 300 as borderline, though labs vary. Borderline results are often checked with homocysteine or methylmalonic acid. Ask your doctor to interpret yours.
Vitamin B12 foods for vegetarians
| Food | Roughly how much B12 |
|---|---|
| Milk, 1 glass (250 ml) | About 1 µg |
| Curd, 1 katori | About 0.5 to 1 µg |
| Paneer, 50 g | About 0.4 µg |
| Egg, 1 | About 0.5 µg |
| Fortified cereals or plant milks | Varies, read the label |
Is spirulina a source of vitamin B12?
No. Spirulina mostly contains pseudo-B12, an analogue the body cannot use, which can even mask a real deficiency on some tests. Do not rely on spirulina, chlorella or fermented foods for B12.
Methylcobalamin vs cyanocobalamin
Both correct B12 shortfall. Cyanocobalamin is stable, cheap and well studied; the body converts it efficiently. Methylcobalamin is an active form and the most common in Indian prescriptions. The form matters less than the dose and taking it regularly.
How much vitamin B12 per day?
The adult requirement is 2.5 µg. Because absorption from a single dose is limited, supplements often contain 50 to 500 µg daily, or 1,000 µg a few times a week. No upper limit has been set, as excess is excreted. A diagnosed deficiency often starts with higher oral doses or injections.
B12 tablets vs injections
High-dose tablets work for most people, even many with absorption problems. Injections are used for severe deficiency, nerve symptoms or pernicious anaemia. Your doctor decides.
Best time to take vitamin B12
Morning, with or without food. Some people find B vitamins mildly stimulating late in the day.
Vitamin B12 side effects
Oral B12 is very safe. Rarely, acne flares or mild nausea. If you take metformin, ask your doctor about checking B12 once a year.
